Sermorelin Versus Exogenous Growth Hormone
Sermorelin is described in the literature as a GHRH analog intended to support the pituitary’s endogenous release of growth hormone in an episodic fashion that aligns with circadian rhythms rather than a bolus administration of growth hormone. The distinction is central to realizing the gains of growth hormone while avoiding potential unwanted effects of exogenous administration; the timing, patterns, and levels may be exerted within physiologic norms without disrupting feedback loops and adjacent hormonal systems.
Timing, Cycling, and Expectations
Conversations with patients should emphasize bedtime administration, cycling use for 5 days on and 2 days off, and use on an empty stomach. This helps to optimize potential benefit by minimizing downregulation of receptors and align growth hormone biology with circadian rhythms. Providers should set realistic expectations that accrue with continued use over months in the context of other drug interventions and lifestyle modifications.

Sermorelin Cautions
Sermorelin should be avoided in pregnancy or breastfeeding as there is insufficient data to recommend use in these subpopulations. Do not use in patients with active cancers or tumors.
